Mission, Vision & Goals

The mission of the non-profit Community Nurse of Thetford, Inc. is to provide residents with health care coordination and care management in situations where they are experiencing uncertainties and difficulties associated with chronic illness, aging, and post-hospitalization adaptation. The intent is to fill gaps in service not currently being addressed by existing health care provider organizations.

Broad Overview

Community Nurse of Thetford, Inc. (CNT) is a nonprofit organization providing support, guidance, and education to individuals and families dealing with chronic illness, aging, or recovery after hospitalization. The community nurse can assist in one or more of the following ways:

  • Coordinate care when several providers are involved
  • Help elders continue to live safely and stay well at home
  • Help follow the plan of care recommended by health care providers
  • Link people with community resources and services to enhance daily life
  • Reduce individual and family stress related to illness and aging

The Community Nurse does not provide hands-on physical care. If skilled physical care or daily living assistance is needed, the community nurse will help the family explore options with home care agencies, hospice care, or individuals in the community. The Community Nurse does not answer emergency calls. The Thetford Fast Squad  provides this service through the 911 dispatch.

Community Initiatives

Health & Wellness

The Community Nurse of Thetford, Vt. serves in a unique role in our community to assist those in need. Anyone is eligible and services are confidential and free.

Examples of assistance are:

  • coordination of care among multiple health care providers
  • teaching and assistance in following health care advice
  • medication teaching and help with organizing medications to reduce error
  • connections to community resources (transportation, senior meals, visiting nurse agencies, etc)
  • family support for issues around aging, chronic illness or end of life
